Job Description Summary
United Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our physician practice team, delivering high-quality, compassionate care in a fast-paced clinical setting. Job Responsibilities:
Perform clinical tasks such as taking vital signs, collecting specimens, and assisting with procedures, always within the LPN scope of practice and under supervision when required
Administer medications safely and accurately, utilizing knowledge of pharmacology and following UHS policies and current clinical standards
Educate patients and their families on health promotion, disease prevention, and management strategies, as directed by a provider or registered nurse
Support and ensure compliance with all regulatory standards including Joint Commission, NYS Department of Health, and infection control protocols
Perform CLIA-waived laboratory testing, including quality control procedures, documentation, and participation in tri-annual AAFP proficiency testing
Maintain accurate and timely documentation in the electronic medical record system to support patient safety and care coordination
Participate in department-specific credentialing and lab validations as required by practice standards and regulations
Collaborate with providers, RNs, and the broader care team to coordinate patient care and promote positive health outcomes
Assist with front office functions such as scheduling, patient intake, and communication of clinical instructions when appropriate
Actively engage in Performance Improvement (PI) initiatives by reviewing data, offering feedback, and implementing changes to enhance care quality
Maintain clinical competency by participating in ongoing training, in-services, and educational opportunities
Uphold a culture of patient-centered care by contributing to a safe, respectful, and welcoming environment for all individuals
Position Qualifications:

Minimum Required:
Graduated an accredited school of Practical Nursing
Current New York State Practical Nurse License, a Limited Permit, or ability to practice within the 90 day grace period allowed by New York State guidelines

Basic Skills Certifications (CPR)
Work Environment
- Work is performed in an office environment and patient care areas which require working in proximity to other employees and patients. Rooms are air conditioned and heated.
- Frequent walking, standing, bending, pushing, and lifting.
- Fast paced environment with frequent interruptions.
- Exposure to equipment cleaning solutions and disinfectants.
- PPE (Eyewear, Masks, Face shields, Shoe covers, and Gloves) as required by UHS Hospitals Infection Control policies.
