Position Overview
We are seeking a knowledgeable and committed Registered Nurse (RN) to serve as a Nursing Home Surveyor/Complaint Investigator in New York, NY. This contract/travel-based role focuses on conducting regulatory surveys, investigating complaints, and ensuring nursing home compliance with state and federal guidelines.

Qualifications
Education & Licensure
- Bachelor’s Degree in Nursing (BSN)
- Active New York State RN license (required)
Experience
- Minimum 1 year of survey experience (required)
- Either:
- 2+ years in utilization review, claims adjudication, medical review, fraud investigation, or monitoring; OR
- 3 years of clinical or administrative nursing experience; OR
- BSN with 2 years of clinical/administrative experience
Technical & Soft Skills
- Proficient with computers and ability to learn new applications quickly
- Strong communication, interpersonal, and analytical skills
- Able to work both independently and collaboratively
Additional Requirements
- SMQT certification is preferred (or must be obtained within 12 months of hire)
- Valid driver’s license and ability to travel for onsite surveys
Key Responsibilities
- Conduct surveys and investigate complaints, including serious allegations (e.g., abuse, neglect) in nursing homes
- Draft comprehensive Statements of Deficiencies (SOD) following Principles of Documentation
- Evaluate Plans of Correction and determine compliance
- Input findings into federal and state databases promptly
- Participate in monitoring during immediate jeopardy scenarios
- Attend required meetings and testify at administrative hearings, when applicable
- Adhere to Department of Health (DOH) protocols and travel to on-site locations as assigned
