Position Overview

We are currently seeking a Licensed Practical Nurse (LPN) to join a reputable skilled nursing care facility in Queens, NY. This position offers the opportunity to provide hands-on care and make a meaningful impact on the lives of residents in a supportive and mission-driven environment.

$47/hour
Full-time schedule with morning and evening shifts
Must be available to work every other Saturday and Sunday

Key Responsibilities

Administer prescribed medications and treatments in accordance with care plans
Monitor residents' vital signs and report any changes in condition
Assist residents with Activities of Daily Living (ADLs) including hygiene, mobility, and nutrition
Collaborate with Registered Nurses, physicians, and therapists to ensure continuity of care
Maintain timely and accurate medical records and documentation
Ensure adherence to infection control policies and facility protocols
Respond promptly to resident needs and emergency situations

Qualifications

Education & Licensure

Graduate of an accredited Licensed Practical Nursing (LPN) program
Active New York State LPN license
Proof of Hepatitis B immunity is required

Experience & Skills

Minimum 1 year of experience in long-term care or skilled nursing
Strong communication and organizational skills
Flexibility to work rotating shifts and weekends
Team-oriented with a compassionate and patient-focused approach
