Position Description

Job Title:  Registered Nurse

Job Summary:

The purpose of Forensic Assertive Community Treatment (FACT) is to deliver comprehensive and effective services to individuals (age 18 and up) who are diagnosed with serious mental illness (SMI) and whose needs have not been well met by more traditional service delivery approaches. These individuals also suffer from co-occurring substance use disorders and have histories of criminal legal system involvement. Assertive Community Treatment (ACT) is an evidence-based practice that provides an integrated set of other evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team to support recipient recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. 

The Registered Nurse provides medication management and administration, health assessment, counseling, and psychoeducation to promote recovery, wellness, and optimal health. The nurse provides at least 80% of services in the community within the framework of core ACT operating principles, professional nursing ethics, and regulatory guidelines. 

Shift Hours:  This position is full-time Monday – Friday from 9:00 AM – 5:00 PM.

Salary:  The salary for this role is $98,000 per year.

Location Address:  151 Lawrence Street, Brooklyn, NY 11201. Our office is easily accessible by public transportation.

Workplace Flexibility:  Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.

What You Will Be Doing:

  • Provides at least 80% of medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers’ health care services, providing psychoeducation to consumers about health and medications, assessing consumers’ mental statuses, rotating on team’s after-hour emergency line. 
  • Completes and signs progress notes, nursing assessments, incident reports, and other required documentation in accordance with the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations. 
  • Collaborate with FACT prescriber and Licensed Practical Nurse (LPN) on consumers’ medication management needs, health care services, maintaining consumers’ medication administration schedules. 
  • Provide directions to LPN regarding medication management and administration services, health education, counseling, and psychoeducation services for clients.
  • Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers.
  • Participates in daily team meetings, clinical meetings, and monthly nurses’ meetings to collaborate and discuss clinical issues and clients’ needs with team members and F/ACT nursing staff.
  • Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services.
  • Any other duties as required by Team Leader and senior clinical leadership. 


What We Are Looking For:

  • Registered Nurse (RN) with current New York State license. 
  • At least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social support. 
  • Extensive knowledge of community treatment, support services, and resources. 
  • Highly organized and self-motivated. 
  • Proficient in computers. 


Preferred Skills:

  • Fluency in Spanish.
