Registered Nurse – Advanced Heart Failure & Transplant (Outpatient)

About the Role:
We are seeking an experienced and highly motivated Registered Nurse to join our Advanced Heart Failure and Heart Transplant outpatient cardiology team at one of New York City’s leading health systems. This is a high-intensity, fast-paced role supporting some of the sickest and most complex cardiology patients in the region. You’ll play a key part in ensuring continuity of care, rapid triage, and clinical stability for patients with end-stage heart failure, mechanical circulatory support (LVADs), recent transplants, or awaiting transplantation.
Key Responsibilities:
Clinical Triage & Urgent Assessment
- Serve as the first clinical point of contact for patients and caregivers calling in with new or worsening symptoms — often in medically fragile states
- Conduct thorough, real-time clinical assessments over the phone: evaluate symptoms like dyspnea, weight gain, orthopnea, edema, dizziness, fatigue, palpitations, and chest discomfort
- Triage urgent calls, provide clinical direction, and escalate care (e.g., direct to ED or same-day provider review)
- Coordinate directly with providers to relay triage information and facilitate urgent interventions
Cardiology-Specific Clinical Competency
- Understand and monitor key cardiac labs and markers including INR, BNP, creatinine, tacrolimus levels, potassium, and renal function trends
- Identify abnormal results and initiate appropriate escalation or follow-up
- Reconcile complex cardiac medication regimens including ACE inhibitors, beta blockers, anticoagulants, immunosuppressants, and diuretics
- Support patient education regarding fluid restrictions, sodium limits, weight monitoring, and transplant medication compliance
Administrative & Operational Duties
- Handle high-volume messaging and faxing, ensuring timely communication with patients, pharmacies, outside facilities, and other departments
- Complete and manage time-sensitive documents including disability paperwork, surgical clearance letters, insurance authorizations, and transplant forms
- Confirm and coordinate follow-up testing, imaging, biopsies, and specialty consults
- Maintain detailed documentation in Epic EMR, ensuring every patient interaction is accurately and promptly recorded
Qualifications:
- Current NYS Registered Nurse (RN) license – Required
- BLS and ACLS certifications – Required
- Experience:
- Minimum 1–2 years of RN experience
- Preferred: Emergency Department, Cardiac Step-down, ICU, or Inpatient Heart Failure/Transplant experience
- Experience using Epic EMR strongly preferred
- Demonstrated ability to work effectively under pressure in a high-volume, high-acuity outpatient setting
- Strong clinical judgment and critical thinking in cardiology-specific scenarios
- Excellent interpersonal, documentation, and communication skills
