Position Description

A healthcare organization in New York City is seeking a licensed Registered Nurse (RN) to join their growing Community Health team in the Bronx.

About the Opportunity:

Schedule: Tuesday to Saturday or Sunday to Thursday
Hours: 8:30am to 5:00pm
Setting: Field
Visits: 4-5 per day

Responsibilities

Conduct in-home and community-based clinical assessments using the nursing process
Complete and manage UAS-NY assessments and ensure compliance with regulatory standards
Collaborate with the interdisciplinary team to develop and update personalized care plans
Administer medications, perform skilled nursing procedures, and monitor patient responses
Educate members and caregivers on medications, self-care, and health maintenance
Coordinate care across various settings, including hospitals, SNFs, etc.
Document all care and assessments accurately and timely

Qualifications:

1+ year of Clinical Nursing experience (CHHA, Acute Care, Medical-Surgical, and/or Critical Care)
Graduate of an NLN-accredited Nursing program
Current NYS Registered Nurse (RN) license
Strong communication, clinical judgment, and documentation skills

Desired Skills:

Bachelor's Degree in Nursing
Experience with Frail or Elderly populations (training provided if needed)
