Registered Nurse- Case Management New

A New York City-based health services organization is looking to boost its Case Management staff in the Bronx with the addition of a licensed Registered Nurse (RN).

About the Opportunity:

• Assignment Length: 13 weeks (possible extension)
• Schedule: Monday to Friday
• Hours: 9am to 5pm
• Setting: Acute Care Hospital
• Department: Care Management

Responsibilities:

• Coordinate and manage patient care from admission to discharge, focusing on ensuring patients receive timely and appropriate care while advocating for their needs
• Develop and implement care plans, facilitate referrals, and monitor patient progress
• Conduct comprehensive assessments, develop individualized care plans, and adjust plans as needed to address evolving patient needs
• Facilitate communication between patients, families, healthcare providers, and insurance companies
• Identify appropriate community resources, facilitate referrals, and ensure timely access to necessary services
• Educate patients and families about their health conditions, treatments, and self-management strategies
• Advocate for patients' rights and needs and collaborate with interdisciplinary teams to ensure quality care
• Maintain accurate patient records and document care plans and progress

Qualifications:

• 2+ years of experience with Case / Resource Management and/or Utilization Management
• NYS Registered Nurse (RN) license
• BLS certification

Desired Skills:

• 3+ years of Care Management experience
• Discharge Planning background
• Experience with Patient Review Instrument (PRI)
• Screen experience/certification
